Release Waiver

Gymnastics and all of its auxiliary and offspring programs are relatively safe activities, especially when considering the safety measures, professional equipment, and high-level training GYMNASTICSETC utilizes and partakes in. Yet, any time activity involves height and momentum, there is a risk of minor to severe injury. In order to make our program as safe as possible, all of our staff members are required to go through our proprietary training courses which include our propitiatory Basic First Response & CPR Certificate program. The course not only trains our instructors in First-Aid but also, through our coaching certificate courses, we create scenarios of potential risk, which instills a safety-conscious philosophy that is applied to our methods of instruction. In consideration of participating in GYMNASTICSETC, I represent that I understand the nature of this Activity and I hereby for myself, my children, my heirs, and executors vow that if any of the aforementioned should partake in physical activity at GYMNASTICSETC, the same shall be qualified, in good health, and in proper physical condition to participate in such Activity. I acknowledge that if I believe event conditions are unsafe, I will immediately discontinue participation in the Activity. I fully understand that this, similar to all other sports and activities, involves risks of bodily injury, including permanent injury, which may be caused by my own actions, or inactions, those of others participating in the event, the conditions in which the event takes place, or the negligence of the “Releasees” named below; and that there may be other risks either not known to me or not readily foreseeable at this time; and I fully accept and assume all such risks and all responsibility for losses, cost, and damages I incur as a result of my participation in the Activity. I hereby release, discharge, and covenant not to sue GYMNASTICSETC, its respective administrators, directors, agents, officers, volunteers, and employees, other participants, any sponsors, advertisers, and, if applicable, owners and lessors of premises on which the Activity takes place, (each considered one of the “RELEASEES” herein) from all liability, claims, demands, losses, or damages, on my account caused or alleged to be caused in whole or in part by the negligence of the “Releasees” or otherwise, including negligent rescue operations and future agree that if despite this release, waiver of liability, and assumption of risk, I, or anyone on my behalf, makes a claim against any of the Releasees, I will indemnify, save, and hold harmless each of the Releasees from any loss, liability, damage, or cost, which may incur as the result of such claim. Furthermore, I hereby authorize GYMNASTICSETC to utilize any photography, video, or other likenesses of myself for advertisement use, promotional use, training education, and/or any other reasonable purposes that GYMNASTICSETC may see fit.

Tuition Cycles: Every Week.
Your tuition will be due every week. We understand proper financial planning is important for our families. We offer a 7 day grace period. After that point, a $10.00 late fee will be applied to your account. Accounts left unpaid for over 30 days may result in automated un
enrollment.

Pro-Ration.
Tuition will possibly be pro-rated the cycle of enrollment if necessary, but no other cycles will be pro-rated, if a camp day is missed due to a holiday/gym closing, you may take advantage of a make-up camp day(s) at a later time. Refunds are not provided.

Required Card on File.
Campers must have a credit/debit card on file in order to be enrolled in summer camp. The card on file will be charged for any unpaid balance on the 1st day of each week. If the client is unable or unwilling to place a card on file, a deposit must be submitted of the value of 7 weeks of camp tuition. This is not refundable. If the client’s account becomes 30 days past due, the client will be dropped from the program.

Payment by Checks.
Check payments or automatic checking account debit transactions with insufficient funds will result in a $35 returned check fee. Returned checks or insufficient bank fund debits exceeding two instances will result in the suspension of check writing privileges/automatic checking account agreement.

Drop Notice Requirement.
"Written Drop Notice" is required to become inactive with GymnasticsEtc (inactive meaning that you will no longer be enrolled in any camps whatsoever). The notice must be received 4 weekdays before the 1st day of the camp week you plan on missing. Please understand that dropping the program means your spot may be filled!

Arrival and Pickup Policy Statement.

The safety of your child is very important to us, please inform us if you know you will be late picking up your participant so we can arrange proper supervision. Our camps are subject to our "Three Tier Security Check-Out System" which states all of the following must occur for the child to be released;
01) The person picking up the child must have their name on the account.
02) The person picking up the child must present a photo ID or staff must be able to recognize the person by face identification from previous pick-ups.
03) The person picking up the child must present the "Safety barcode" on file or staff must be able to recognize the person by face identification from previous pick-ups.

Ensure you give the front desk all people who potentially may pick up your child from camp so their name can be placed on file. Otherwise, the child will not be released from camp without further security steps, which can be time-consuming.

Medication and Illness

Should the parents desire GymnasticsEtc to administer their child(ren)'s medication, a written consent form must be completed by the parent and given to GymnasticsEtc. GymnasticsEtc may refuse to give medication. The medication must be in a proper container with the child's name and dosage information clearly marked.
If the child(ren) has suffered a serious accident or illness within the past twelve months, is subject to a serious health condition, or there is any question about activity restriction, further information for the child(ren)'s doctor or specific permission to participate may be required at the discretion of the Director of GymnasticsEtc. GymnasticsEtc staff and volunteers are not qualified to care for some children with special needs. GymnmasticsEtc will only accept those children after evaluation.

Injuries

In the event that a camper suffers any illness or injury requiring emergency treatment while involved in GymnasticsEtc. activities, I hereby give my permission for any necessary hospitalization or medication on the recommendation of medical personnel, in which case I will be responsible for all costs. By signing this document, either individually and/or in the capacity of a natural or legal guardian, you acknowledge the inherent risks of bodily injury, psychological injury or even death, in the activities of gymnastics, swimming and any other activities GymnasticsEtc. may transport my child to as part of the camp or after school care program, as well as through presence as a spectator. By signing this document, you either individually and/or in the capacity of a natural guardian, hereby release, hold harmless and exculpate GymnasticsEtc. Inc., it's officers, agents, representatives, and employees from any and all liability for their negligence in allegedly bringing about bodily injury, psychological injury, or death. Your release of liability for negligence, set forth above, further extends to any defective condition of the premises whether or not known to occur off-premises during transportation to or from related events.

Transportation
I give my permission for my child to participate in all GymnasticsEtc activities for the time period indicated, including those activities held away from the regular activity sites. I understand that GymnasticsEtc will inform me as to off-site activities and locations prior to my child(ren)'s participation. I will be responsible for transportation to and from activities or am willing to permit GymnasticsEtc to select transportation methods of my child.
